REGISTER
WITH THE PRACTICE?
Anyone wishing
to register with the practice should, in the first instance, contact our receptionist
to request patient registration forms. Individual forms are required for each
person wishing to register. All newly registered patients are asked to fill in
a questionnaire and have a consultation with the healthcare assistant within
three months of registering.
Patients have a right to express a preference to receive services from any of
the listed health care clinicians and the practice will endeavour to comply with
any preference expressed. However, the preferred clinician reserves the right
to refuse to provide services to the patient if they do not routinely perform
the services in question.
TEMPORARY PATIENTS
Anyone domiciled in the practice area who requires medical attention may
register with the practice on a temporary basis. Patients doing so require to
be in the area for a minimum 24-hour period and can remain on a temporary basis
up to a maximum three month period.

If you change your address or name please let us know. Patients moving outside the practice area will require to register with a new medical practice closer to their new place of residence.
